The Client who takes this test voluntarily consents and authorizes ICM Medical Group (“ICM”) to receive, test, and analyze the self-obtained specimen for the purposes of a COVID-19 diagnostic test. The Client acknowledges that the nasal or oral swab obtained was done appropriately, after learning the process from the video links recommended by ICM as well as under the guidance and supervision of ICM's clinical staff via FaceTime. The Client understands that there are risks and benefits associated with undergoing a diagnostic test for COVID-19 and there may be a potential for false positive or false negative test results. The Client assumes complete and full responsibility to take appropriate action with regard to the Client’s test results. Should the Client have questions or concerns regarding the result of the test, the Client shall promptly seek advice and treatment from Client’s own medical provider. The Client acknowledges that ICM’s role is strictly limited to COVID-19 testing. ICM’s responsibility is to test the specimen sent by Client for COVID-19 and to provide the result. The Client understands that ICM is not acting as Client’s medical provider; this testing does not replace treatment by Client’s medical provider.
ICM Medical is offering this test using equipment and PCR test methods approved under the FDA Emergency Use Authorization. This test has only been authorized by FDA for detection of nucleic acid from SARS-CoV-2 (i.e., the COVID-19 virus), and not for any other virus or pathogen. It is only authorized during the duration of the COVID-19 emergency declaration by federal regulators. ICM Medical may modify or cease to offer the test and the test kit upon direction of state or federal regulators in its sole discretion.
